Online Application

1. Fill out and submit the Volunteer Application online.

2. Print a copy of your DMV 3-year Driving Record from www.dmvnv.com to bring with you to our office. Here's how:  in the mydmv section, click on: driver history printout.

3. Download, print, and sign the Fingerprint Background Waiver.

4. Download, print, and sign the State Background Check form.

5. Download, print, and sign the Family Court and Services Background Check Request form.

6. Download, print, and sign the National Child Protective Act form.

7. If you do not have access to a printer, you may complete steps 2-5 at our office.

8. Contact us at 775-505-2272 to schedule an appointment to come to our office to complete the application process.

Please be sure to bring the following items with you to our office to complete the application process:

• Your current Driver's License

• Proof of current auto insurance

• DMV 3-year Driving Record

• Fingerprint Background Waiver form

• County/State Background Check form

• Family Court and Services Background Check Request form

Please note: there is a charge for a background check that is not reimbursed by Advocates for Children of Rural Nevada - a 501(c)(3) nonprofit organization.
